    Yes, a printable version of the Canada specific requisition form is available in both English and French at www.athenadiagnostics.ca.
     
  2. What happens to the patient’s specimen if the Ministry of Health letter of approval is not sent with the specimen?
     
    Athena Diagnostics holds all specimens until it receives Ministry of Health prior approval or other payment. Athena will follow-up with the Ministry of Health if we know that an approval is pending. Testing begins once payment is received.
     
  3. How can Athena Diagnostics help me with the cost of shipping these samples from Canada?

  4. Who is responsible for patient specimen collection?
     
    Specimen collection is the responsibility of the physician and patient. Athena Diagnostics is not responsible for specimen collection logistics assistance and does not cover any specimen collection fees.

  6. My patient is not eligible for MOH coverage, how else can the test be paid?
     
    In the event that neither the MOH nor the healthcare facility will pay for testing services, the patient can pay out of pocket. Payment can be made via credit card, bank check or wire transfer, but must be denominated in US dollars. For details, please click here.
